NextGen Sports Club policies
Waiver, Release & Medical Consent
Last updated: October 25, 2026
Every player's parent or legal guardian accepts this agreement when registering with NextGen Sports Club (“NextGen”) through PlayMetrics. It applies to all NextGen practices, games, events and other NextGen activities (“Activities”). In this agreement, “I” means the parent or guardian signing for themselves and for the player.
1. Risks of Soccer
I understand that soccer is a physical, contact sport. Risks include collisions, falls, being struck by the ball, heat illness, sprains, fractures, concussions and other head injuries, and in rare cases permanent disability or death. Risks can come from the player's own actions, from other people, from the weather, and from the condition of fields, courts and equipment.
I have considered these risks, I confirm the player is physically able to take part, and I choose to let the player participate.
2. Health Information
I will tell NextGen in writing, through PlayMetrics or by email, about any medical condition, allergy, medication or injury that could affect the player's safety, and I will keep this information up to date.
3. Medical Treatment
If the player is injured or becomes ill and I cannot be reached, I authorize NextGen coaches and staff to give basic first aid and to arrange emergency medical care, including transport to a hospital, from licensed medical providers.
I am responsible for the cost of any medical treatment.
4. Concussions and Return to Play
A player with a suspected concussion is removed from play and does not return that day. The player may return to NextGen Activities only after written clearance from a licensed health care provider. I agree to follow this rule and to tell the coach about any head injury that happens outside NextGen.
5. Release of Claims
To the fullest extent permitted by law, I release NextGen Sports Club and its directors, coaches, staff, volunteers and agents (the “NextGen Parties”) from claims for injury, illness, loss or damage arising from the player's participation in Activities.
This release does not apply to gross negligence or intentional misconduct, or to any claim that applicable law does not allow to be released.
6. Indemnity
To the fullest extent permitted by law, I agree to reimburse the NextGen Parties for losses, costs and reasonable attorney's fees caused by the player's or my own conduct, including claims made by other people.
7. Rules and Conduct
The player and I agree to follow the NextGen Code of Conduct, the Player Safety & SafeSport policy, coaches' safety instructions, and the rules of the leagues and organizations NextGen belongs to.
8. Photos and Video
NextGen takes photos and video at Activities for coaching and to share the club's activities. Our Privacy Policy explains how we use them and how to opt out.
9. General Terms
- This agreement is governed by the laws of the Commonwealth of Virginia.
- If any part of this agreement is found unenforceable, the rest stays in effect, and that part is enforced to the greatest extent allowed.
- This agreement stays in effect for every season the player is registered with NextGen, until I withdraw the player in writing.
- I confirm I am the player's parent or legal guardian and that I have read and understood this agreement.
